Clinical Support Programs Scheduler

Job Description:

The Clinical Support Programs Scheduler provides coordinated scheduling for the Nursing Assistant Program and the Crisis Care Program.

 

QUALIFICATIONS:

1. Two year business or related degree desirable.

2. Ability to use computer adequately to meet agency needs. 

3. Keyboarding skills and knowledge of Word and Excel.

4. At least one year of office or scheduling experience or the ability to learn. 

5. Ability to communicate well both orally and in writing.

6. Ability to work within an interdisciplinary team. 

7. Ability to be flexible and possess critical thinking skills.

 

DUTIES/RESPONSIBILITIES:

1. Coordinates all staffing for the clinical support programs offered by the agency including the nursing assistant program and crisis care.

2. Receives referrals for aide services from the RN Case Manager and assigns appropriate aide. Provides entry of RN assigned plan of care. Prints, reviews and distributes to assigned aide.

3. Coordinates with all nursing assistants regarding daily scheduling of patient visits.

4. Insures that the aide frequency is met in accordance to the patient’s plan of care.

5. Collects all aide documentation and confirms that all documentation has been submitted for each visit scheduled.  

6. Responsible for verification of data entry into Netsmart medical software.       

7. Notifies the Clinical Team Leaders or RN Case Manager of any variance to the aide schedule that impacts the patient’s plan of care. Communicates effectively with supervisors.

8. Coordinates with the Clinical Team Leaders to provide staffing for the Crisis Care Program. Insures that all Medicare/Medicaid guidelines are followed in regard to appropriate nursing and aide hours of care.

9. Opens patient’s chart upon admission, ensuring all required documents are present, according to applicable regulations and agency policies and procedures. 

10. Prepares chart for closure at patient’s death or when discharged

11. Assists with daily distribution of medical supplies.

12. Assists with switchboard coverage as needed.

13. Attends monthly clinical staff meetings and prepares minutes.

14. Participates in staff training and educational programs.

15. Performs other duties as assigned.

16. Adheres to the organization’s Standards of Conduct and HIPAA regulations.

17. Reports any potential misconduct or violation of the organization's Standards of Conduct, HIPAA regulations, or federal or state laws and regulations.

Part-Time Assistant Volunteer Coordinator

Job Description:

Hospice of Randolph County is looking for a Part-Time Assistant Volunteer Coordinator. This newly created position is responsible for assistance with recruitment, training, placement, retention, and supervision of agency volunteers according to agency policies and procedures and applicable regulations. Responsibilities include, but are not limited to, assuring compliance of volunteer services with all applicable standards, regulations, policies and procedures; assisting in evaluating volunteer services/program for continuous quality improvement; working to promote a positive public image for Hospice of Randolph County programs through volunteer involvement and services; participation in public speaking engagements and other opportunities to recruit volunteers; preparing and completing accurate documentation and reports as needed.

Qualifications:

•Two year degree preferred. High school diploma required.

•Experience in volunteer management; knowledge or the ability to become knowledgeable of the hospice philosophy of care. 

•Excellent communications skills, both oral and written, required. 

•Ability to enter and retrieve data on computer.

•Ability to work in an interdisciplinary team setting, as well as independently.

•Ability to work flexible schedule as needed.

•Ability to learn regulatory requirements for volunteer program.

•Ability to provide transportation as needed.  

•Valid North Carolina driver’s license required.

•Ability to organize and implement multiple tasks
