*Free* Advance Care Planning Clinic on Sept. 28Posted on Aug 7th, 2019 | Perma Link
In life, we prepare for many things including college, marriage, children and retirement but we rarely begin the conversation about end-of-life wishes. According to a Reuters Health study, most individuals have preferences about end-of-life treatment but only 37% of Americans have documented their healthcare wishes by completing a Living Will and/or Healthcare Power of Attorney. In an effort to empower our community to plan ahead, Hospice of Randolph County will provide a FREE Advance Directives Clinic for the public on Saturday, September 28. The Advance Directives Clinic will take place in the Education Building at Hospice of Randolph County (416 Vision Drive, Asheboro, NC) with sessions at 9am, 9:30am, 10am and 10:30am. During the half-hour sessions, attendees will receive free assistance with the completion of Living Will and Health Care Power of Attorney NC Statutory Forms. No legal service, legal advice or customizable forms will be offered.
This event is free and open to the general public. Seating is limited and pre-registration is required. To pre-register, please contact Hospice of Randolph County at 336.672.9300.
Participants must bring a driver license or photo ID as well as the name, address and phone numbers of the individuals that will be assigned as the primary and secondary healthcare agents.
