Caring for someone with dementia can present a unique set of challenges. Learning more about your love one’s disease, understanding the various changes they may be experiencing and keeping your own health in mind are all important parts of caregiving.
We want to help support you in your care journey.
Our Care Connection Dementia Care program:
- Helps coordinate and organize healthcare services.
- Provides in-home care and support resources.
- Gives access to registered nurses, social workers, community health workers, after-hours on-call support, and as-needed visits from a medical provider.
- Delivers support directly in the home which helps reduce unnecessary transitions of care.
- Offers caregivers a level of support they may not currently have.
- Educates caregivers on changing needs of someone with dementia and how to adapt.
Who can participate:
- Anyone with a diagnosis of dementia
- Traditional fee-for-service Medicare – Parts A & B – beneficiaries
Who cannot participate:
- Individuals with Medicare Advantage
