Hospice Care in Nursing Facilities

For individuals residing in skilled nursing facilities, continuing care retirement centers, or assisted living facilities, the ability to receive hospice care where they live is a gift.

With hospice support in care facilities, Hospice of Randolph and Hospice of the Piedmont team members partner with the facility to provide comfort and support.  The hospice team is composed of compassionate professionals who, in partnership with the facility staff and medical director, address the physical, emotional and spiritual needs of patients.

These special teams from Hospice of Randolph and Hospice of the Piedmont are available to partner with facility caregivers to ensure that eligible residents receive high-quality care in such areas as:

  • Symptom management,
  • Emotional and spiritual support for residents and their families, and
  • Additional monitoring to complement nursing facility care.

Our teams are also advocates for honoring patient wishes.

Referrals for nursing facility hospice care may be initiated by the resident, their family or the facility staff.  The patients′ primary care physician must agree that they are appropriate for hospice care before services can be initiated.

Hospice services in nursing facilities are reimbursed by Medicare, Medicaid, and most private insurance coverage. With generous community support, no one is turned away or denied care because of financial resources.
